Each of the following Intake Forms are available for you to download, print and complete and then bring them with you to your first visit.

When you click on the name of the form, the pdf-document will be displayed in your browser window along with a print icon in the upper right portion of the window.

  1. Client Registration–required for ALL first visits
  2. Pain Assessment–required for each visit
  3. Notice of Privacy (HIPAA)–for your information
  4. Client Insurance–required if the treatment is covered by your insurance
  5. Lien–required if your treatment is due to an auto accident
